De Ceuninck et al. address the question of why osteoarthritis biomarkers are so important, with special emphasis on the following: the criteria that a biomarker candidate should fulfill for validation in osteoarthritis prognosis and diagnosis; identification hurdles; and recent research studies giving reasons for hope.
